NATURE OF POSITION: The Director of Quality and Risk Management is an integral member of the interdisciplinary health team and assumes responsibility for the standard of nursing care delivery in the clinic setting.

MINIMUM QUALIFICATIONS:

· Bachelor’s Degree in a health-related field; preferred

· 5-7 years’ experience in a clinical setting; required

· Strong clinical and teaching skills

· Able to assume a leadership role in a team environment

· Strong interest in working with an interdisciplinary health team and culturally diverse patient population

CLASSIFICATION: Exempt

SUPERVISION EXERCISED: Quality Management Staff

SUPERVISION RECEIVED: Chief Medical Officer

RESPONSIBILITIES:

· Patient Complaints – receive, distribute to appropriate department head, collect prudent data, suggest corrective action team if indicated, track results for improvement

· UDS – track clinical measures throughout the year: health outcomes and disparities, selected dx and services rendered and EHR capabilities and Quality recognition, and other data as directed

· Acts as first line EHR “Super User” for medical/nursing staff to help in problem solving and workflow analysis

· Patient satisfaction surveys – create, distribute, examine for improvement opportunities, and track results if indicated

· Incidents (Patient) – receive report, distribute as appropriate, receive corrective action as appropriate, and track the implemented change if necessary

· The Joint Commission (TJC) – Ambulatory Care only (application, updates, information)

· Meaningful Use – Oversight and implementation of the requirements for collecting and reporting data, and training of staff on required measures/changes

· Collaborations – liaison with agencies to collect and retrieve data, submit results, educate staff regarding processes and guidelines

· QI/QA committee coordinator – oversight of QI/QA plan, work with CMO and other departments as necessary, collect data, track implemented corrections, monitor process changes

· Infection Control – oversight and monitoring of plan and processes, education, monitoring of compliance with plan

· Risk Management – oversight and monitoring of the Risk Management Plan working with other department directors and Compliance Advisor as necessary

· Patient Centered Medical Home (PCMH) – Responsible for understanding and educating staff, patients, and families on the PCMH objectives

· All other duties as assigned
