Supervision (Given and received)
- Given: The Director will supervise the professional staff of RN’s, LPN’s and Home Health Aides and Staffers. Type of position and staff numbers are determined by needs and may vary.
- Received: The Director works under the direction of the Executive Director. The employee is responsible for the method in which to complete their work.
Job Responsibilities and Duties (Essential Responsibilities)
Management
- Organize, direct and evaluate the operations of our home care agency, including budgeting, staffing, supplies and equipment, including adjusting staffing needs as necessary to meet the clients’ needs.
- Develop, maintain and update as necessary written policies and procedures that govern the daily functions of the agency, consistent with current nursing standards and evidence-based practice. Provide clinical leadership to licensed staff.
- Develop relationships with other service providers, such as physicians, pharmacies, clinics, hospice providers, therapy services and establish protocols for coordinating their services for our clients receiving our home care service and to ensure smooth transitions for clients who may need hospitalization or a move to a nursing home or another setting.
- Develop atmosphere of teamwork with all departments represented.
- Respond timely and appropriately to emergency situations.
Financial
- Prepare and implement budget; monitor throughout the year ensuring financial integrity.
- Responsible for overseeing monthly invoicing and collection for all services performed.
- Responsible for semi-monthly payroll review, input and approvals.
- Review care levels and a la carte services annually to determine changes in pricing.
Resident Care
- Develop, implement and oversee our agency’s quality management and quality improvement program, analyze resulting data and make changes necessary to improve client outcomes and agency operations.
- Establish and maintain proper medical documentation regarding patient care.
- Ensure that nursing staff provides on-going evaluation of patient care needs.
- Establish and maintain positive patient and family relations, e.g., effective communications, manage patient and family concerns, problem solve, etc.
- Participate in Manager-on-Duty on-call rotation.
- Ensure that the professional staff is developing a plan of care, initiating, coordinating and prioritizing referrals and services for each client.
Compliance
- Establish the agency’s policies regarding the types of services we will provide, the, the types of reimbursement our agency can accept and other policies regarding the scope of our services.
- Periodically audit client records and supervise staff to assure that staff are following agency procedures, are providing quality services and are maintaining client records according to agency policy and state home care requirements.
- Is responsible for overseeing the implementation of the agency’s infection control plan and TB prevention plan by the RN.
- Orients new RNs and LPNs to the organization’s policies and procedures.
- Is available for on-call medical questions and/or emergencies via phone and to fill in for the RN due to illness or vacation.
- Responsible for ensuring that any issues identified in a licensure survey are appropriately address, either though changes in policies, procedures, training, etc. Responsible for contacting MDH regarding any requests for reconsideration or appeals of survey findings that appear to be undeserved or inappropriate.
- Responsible for compliance with the reporting requirements of the Vulnerable Adult Act and the Board of Nursing, including the investigation and immediate reporting of incidents of suspected client maltreatment to the Common Entry Point and the documentation and reporting of the findings of such investigations.
- Responsible for investigating complaints about our services submitted by clients, clients’ representatives or others and identifying steps to address the complaint and communication with the clients and the clients’ representatives about our agency’s actions to resolve the issue.
- Comply with HIPAA regulations by maintaining and safeguarding patient and staff confidentiality.
- Maintains current knowledge and ensures compliance with applicable licensing requirements, laws and regulations and make sure staff is appropriately informed of new changes.
Human Resources
- Provides direct supervision of RN Case Managers and LPNs within 30 days of date the staff person begins work and thereafter provides direct supervision on a schedule consistent with the RN’s experience. Completes the RN’s and LPN’s annual performance review.
- Responsible for hiring, disciplining and discharging staff, determining pay rates and awards, developing and implementing employee recognition programs and other personnel matters, consistent with state and federal employment law.
- Assure that staff are trained consistent with OSHA requirements regarding Employee Right to Know, bloodborne pathogens, Material Safety Data Sheets, etc. and that they follow agency safety rules and regulations. The Home Care Director is responsible for identifying and eliminating to the extent possible any safety hazards for staff and for reporting and logging all work place injuries.
- Ensure that agency staff has adequate equipment, including protective equipment, that is needed to perform their jobs and that the equipment is in good condition.
- Ensure orientation, staff development and continuing education programs are available to all staff and work with nursing staff to implement staff education.
- Provide in-service opportunities for staff to maintain requirements for licensure and certification.
- Schedule and create agenda for monthly staff meetings. These may be scheduled before in-service training.
- Attend staff meetings, in-service education programs and care conferences as appropriate.
Marketing
- Shop competition to ensure we are competitive in the market.
- Represent StuartCo, the properties and SPHC in the best light.
- Participate in community and professional associations and meetings as appropriate.
Job Standards
- Graduate of an accredited school of nursing. Must be registered and licensed with the State of Minnesota as a RN, and must maintain current unencumbered license with the Board of Nursing.
- At least 10 year(s) experience in home care or long-term care or public health] and general nursing experience.
- 3-5 years supervisory and management experience.
- Working knowledge of the state requirements for the Comprehensive Home Care license, federal and state reimbursement programs used by our clients.
- Excellent judgment and decision-making abilities and problem solving skills.
- Ability to incorporate up-to-date evidence-based research into our agency’s procedures and protocols.
- Ability to develop and monitor the agency’s budget.
- Ability to develop and implement a quality management and quality improvement process for our agency and to gather, track and analyze data related to client outcomes and other key measures.
- Ability to organize and prioritize work projects.
- Ability to maintain personnel, financial, and other confidential information.
- Act as StuartCo ambassador.
- Possess the ability to deal tactfully with residents, owners, staff, other managers, vendors, and the general public.
- Follow and adhere to organizational policies and procedures.
- Must possess a valid Minnesota driver’s license, acceptable driving record, and access to a vehicle.
- Able to read, write, spell, do basic math, speak and understand English.
- Maintain neat, well-groomed, professional appearance.
- Must not pose a direct threat to the health or safety of other individuals in the workplace.
Physical Requirements (based on essential responsibilities)
Must be able to continuously perform tasks such as but not limited to those listed below:
- Perform tasks requiring action of muscles or groups of muscles such as walking, stooping, standing and squatting.
- Use hands and arms for difficult manipulation to use a computer/typewriter keyboard, various office equipment, and basic medical equipment.
- Perform tasks which require foot and/or hand-eye coordination such as typing, driving a car, and basic medical procedures.
- Speak clearly; make self-understood and articulate with extreme accuracy in face-to-face and telephone conversations while communicating with employees, patients, corporate office, and property owners; ability to communicate effectively in writing.
- Hear normal sounds and voices with some background noise to answer phones, communicate with patients and others.
Must be able to frequently perform tasks such as but not limited to those listed below:
- Push and/or pull objects over 10 pounds; lift and/or move objects over 25 pounds.
- Sit and stand for prolonged periods to perform office work, attend meetings, etc.
- See distant objects sufficiently to pass the Minnesota driver’s license vision test.
- See objects closely to read directions, reports, patient files, etc.
Must be able to occasionally perform tasks such as but not limited to those listed below:
- Push and/or pull, lift and/or move objects over 50 pounds.
- Use hands and arms for simple grasping, pushing and pulling as needed.
- Perform tasks which require steadiness of motion.
- Climb stairs, bend to the floor, kneel, and twist.

- Concentrate on fine detail with constant interruption with minimal errors.
- Ability to perform duties under a variety of stressful and/or emergency situations.

- Remember multiple verbal and written tasks/assignments given at the beginning of a period extending over long duration of time (months).
Equipment
Must be able to operate:
- Microcomputers including laptop/tablet, office equipment, telephones and cellular telephones, patient call systems, door alarms, life lines, pagers/walkie-talkies, oxygen equipment, vital signs apparatus and other medical devices.
- Motorized vehicle.
Environment
May be exposed to communicable diseases, infection, airborne and blood borne pathogens, toxic fumes and cleaning and other chemicals.
Working Hours
Typically Monday through Friday, 8 hours per day.
