Associate Case Manager

Schedule: Full-Time 8am-4:30pm
Reports To: Director of Case Management
Summary: This role incorporates dual responsibilities in the area of Discharge Planning and in Office Management. Under the supervision of the case manager works with the patient/family, multidisciplinary team and physicians, to optimize the process of patient’s progress through the continuum of care…intervening and executing the discharge plan.
Requirements: Experience working with computers, High School Diploma or equivalent with college course or experience related to basic office procedures. Working knowledge of medial terminology.
Preferred: Discharge Planning Certificate or Equivalent experience.
