Summary:
This position works under the direction of the Director Population Health to provide direct project management support to assigned projects within the organization. The Project Manager initiates, plans, develops, executes and manages project timelines, conducts problem assessments, ensures efficient project implementation, and reports on project performance in conjunction with project sponsors. Expertise in the use of project management tools, facilitation, group processes and critical thinking is essential. The Project Manager works to ensure maximum effectiveness of resources, quality, change, integration, cost, and scope management using effective communications techniques. The Project Manager identifies opportunities for efficiency, effectiveness and process innovation while analyzing relevant project data and implementing process improvements.
Principle Accountabilities:
- (60%) Provide direct project oversight by initiating, planning, executing, controlling and closing out projects that drive quality improvement and high-reliability as assigned by the Director Population Health.
- Project Management. Developing a work breakdown structure and project oversight to ensure project deliverables and outcomes are met in a timely manner. Monitors progress of projects against schedule, tracks budget, reports on barriers, issues, and identifies mitigation strategies to manage risks.
- Planning. Establishes and documents charters, governance, project plans, project schedule, clearly identifying tasks, deliverables, assigned roles and responsibilities and define the project’s critical path. Assists with development of success metrics to effectively report project results.
- Resource Management. Directs project teams through project completion. Identifies appropriate resources required, vendor oversight, ensures engagement of teams, secures commitment to the work, tracks budget, and escalates to stakeholders as resource needs are identified.
- Communications. Communicates timely and effectively by developing a communication plan and status reports on progress of assigned projects. Regularly updates project sponsors, executives and stakeholders. Schedules meetings, prepares agendas, and facilitates cross-functional project teams.
- Scope & Change Management. Ensures the agreed upon scope as defined in the project charter is delivered to the requestor. Escalate changes to scope, schedule or budget by effectively utilizing the change management process.
- (20%) Provides direct support and project oversight for St. Luke’s clinical practices and departments undergoing improvements in systems and delivery of care.
- (20%) Performs other duties as assigned.

Knowledge, Skills and Abilities:
Required Experience:
- Minimum of two (2) years of experience in healthcare or public health.
- Minimum of two (2) years of experience in project management
- Knowledge of quality improvement concepts and healthcare transformation models, e.g., Accountable Care Organization (ACO), Patient Centered Medical Home (PCMH), Comprehensive Primary Care (CPC).
- Proven track record of successfully leading teams and implementing change initiatives required.
- Experience managing teams from cross-functional departments and disciplines required.
- Experience leading complex and concurrent projects with various size teams preferred.
- Must be able to work independently and collaborate with teams of individuals in diverse settings with a solution-oriented approach.
- Must have excellent relationship management skills, data analysis and management skills and communication, coaching and organizational skills.
- Proficiency with Microsoft Office and project management tools/techniques and methodologies is required.
- Experience with electronic health record.
Required Education:
- Bachelor’s degree in healthcare or related field.
Licenses or Certifications:
- PMP, Lean Six Sigma, or CPHQ certification preferred.
Working Conditions:
Work will be completed in both an office and hospital/clinic environment. Schedule may vary depending on assigned rotations. Work will occur at all St. Luke’s sites and will require travel between sites.
Hearing and Speaking: Continuously for or in-person and telephone conversations.
Standing: Frequently, as required to make presentations or perform other job tasks.
Sitting: Continuously, chair or stool up to 8 hours per day – able to intermittently move about.
Changing Position: Frequently, as needed from sitting to standing to walking, etc.
Walking: Occasionally, 2 – 4 hours per day on concrete, vinyl and carpeting.
Carrying: Up to 25 pounds (supplies, equipment, books, papers, etc.) for intermittent period and distances.
Lifting: Up to 25 pounds (patients, equipment, supplies, books, etc.) Intermittent throughout the day.
Pushing/Pulling: Up to 25 pounds, intermittent, throughout the day.
Bending: From the waist and knees, frequently throughout the day.
Reaching/Handling: Fine manipulation, repetitive, gross motor requiring reasonable range of motion in upper extremities, including overhead reaching.
Twisting: Reasonable range of motion required, intermittent periods, throughout the day.
Climbing: Staircase, occasionally throughout the day.
Crawling: Infrequent but possible on vinyl, carpet surface to retrieve or attend to articles that require attention.

The most significant of duties are included but this does not exclude occasional work assignments not mentioned or developmental duties.
