Summary:

The Population Health Project Manager supports the priority population health initiatives of Howard County General Hospital (HCGH). This position is part of a team created to manage the Howard County Regional Partnership – a multi-faceted intervention framework spanning the acute, post-acute and primary care settings to improve health outcomes for complex, chronically ill Howard County Residents. The Project Manager will: 1) establish and track project plans; 2) coordinate information sharing and work products; 3) develop and maintain systems that support regular reporting requirements to internal and external stakeholders and funders; and 4) collaborate on short-term projects and process improvement efforts.

Requirements:

Bachelor’s degree in a field of human services required, Master’s degree in a health care administration, public health or closely related field preferred.

Requires minimum of two to four years of progressive experience in project management and demonstrated understanding of project management principles. Additional education may be substituted for experience. Project Management Certification is highly desired.

Experience working with health systems, hospitals, or community-based health care or public health organizations preferred. Ideal candidate will have experience with managing reporting elements tied to projects supported by external funders.
