Broward Nursing & Rehabilitation Center is seeking a Social Services Director.

 Maintain Regulatory compliance related to Social Services
 Provide services to residents and their families, identify social and emotional needs, problem solving
 Discharge Planning
 Maintain care conference schedules
 Full documentation requirement for social services
 Completion of MDS as related to social services
 Coordination of services, community and facility based.
 Work collaboratively with interdisciplinary team to coordinate plans of care
 Conduct and document social services evaluations, including documentation of problems/needs.
 Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
 Keep resident families informed of resident problems, concerns, needs, transfers and changes.
 Maintain a list of community services
 Demonstrate respect and compassion in every interaction.
 Conduct oneself with the highest degree of honesty and integrity in every interaction.

Bi-lingual preferred
MSW preferred with 2 years SNF experience
BSW with 5 or more years SNF experience
Knowledge of Point Click Care, MDS
