Summary:
The Jelani House Residential Program will be a new emergency housing project funded through the City of San Francisco and managed by the Homeless Prenatal Program. Jelani House will provide a safe, stable living environment for up to 17 homeless pregnant and postpartum individuals in San Francisco. With 30 years of experience serving this population, HPP will provide intensive case management, perinatal education and other support to help individuals have healthy pregnancies, transition into parenthood and permanently end homelessness.

The Jelani House Associate Program Director is responsible for the direct management of the Jelani House Residential Program and the facility. This position is responsible for the implementation and oversight of the Jelani House intensive case management program as well as the operations of the facility. It includes direct supervision of the Assistant Program Manager/Residential Coordinator, Jelani House Case Managers, Nutrition Manager, and Health Educator, and indirect supervision of Community Clerks, Janitor, Contract Maintenance provider, and interns as required, in addition to providing a level of direct service as determined by the Deputy Director.

The Jelani House program provides residential services and intensive case management and health education services to at-risk homeless pregnant individuals who may be experiencing behavioral health issues, substance use, domestic violence or other challenges. Participants may also be transitional aged youth. This position requires strong people and program management skills and previous management experience.

This position will require compliance with all applicable laws and regulations; will maintain acceptable occupancy levels and develop a supportive environment for all tenants.

Essential Duties and Responsibilities:

The Associate Program Director plans and administers all aspects of the Jelani House Program; directs and coordinates various activities within the program; coordinates with the City’s Coordinated Entry intake/referral department to ensure smooth and effective admission processes for new clients and high utilization of the facility; ensures facility is operating in accordance with the goals of the contract; and ensures well being and safety of program participants. Provides direct services such as intake, assessment, crisis intervention and case management as needed and has ultimate responsibility for retention and completion of all clients. Majority of work will take place onsite at Jelani House residential facility with weekly off-site meetings at Homeless Prenatal Program main facility and off-site meetings with city agencies and providers as needed. This is a full-time exempt level position requiring weekday shifts and occasional evening or weekend hours and on-call responsibilities typical for a residential program operating 24/7/365.

Supervision and Oversight

  • Supervises staff, providing case management expertise and feedback as well as establishing goals and assessing skill development needs for team members.
  • Perform regular weekly supervision with all direct reports. Supervision with Case Managers includes review of client cases to ensure best practices in case management and client care.
  • Ensures that Jelani staff are aware of all contract objectives and outcomes as well as HPP initiatives and procedures.
  • Evaluates the performance of supervisees by conducting annual written performance evaluations as well as an informal progress performance evaluation mid-year.
  • Collaborate with Wellness Services Program Manager to ensure that clients are receiving the best care and to facilitate full access to Wellness Services.
  • Facilitate weekly team meetings and co-facilitate quarterly joint Jelani House, New Beginnings, and Wellness Services team meeting.
  • Manage Jelani budgets, including facilities, groups and classes, staff training and client services items (vouchers, gift cards, tokens).
  • Maintain contact and referral information for substance abuse treatment programs for pregnant and parenting women in San Francisco.
  • Supervise MSW Case Management Interns.
  • Direct services as required.

Staff Development

  • Manage to agency, and position, core competencies, providing guidance and skill building to staff.
  • Ensure that all Case Managers are trained and capable of addressing issues of pregnancy, substance use, domestic violence, mental health, child welfare and housing.
  • Ensure that staff are trained and able to use their skills in motivational interviewing, counseling, skill building, and strengthening of protective factors to achieve desired client outcomes of increased family functioning, knowledge of parenting/child development, a healthy pregnancy and birth, knowledge about family planning options, increased financial/life skills, and resolution of CPS cases or family reunification.
  • Responsible for the on-going professional growth of staff including preparing and managing to individual development plans, determining training needs, and arranging appropriate training.

Program Management and Administration

  • Collaborates with the Director of Programs, Associate Director of Programs – Clinical Services, and Director of Evaluation in the design and implementation of the Intensive Case Management and Jelani House program service model. This includes: developing service goals and objectives, developing tools and approaches for staff, testing new practices, and refining the model as needed.
  • Collaborates with Director of Programs, Associate Director of Programs – Clinical Services and Wellness Services Program Manager to develop comprehensive programs for prenatal and post-partum clients addressing health and wellness, prenatal education and child development.
  • Attends program meetings and meetings with other participating agencies as required.
  • Maintains ongoing collaboration with San Francisco city agencies including Department of Homelessness and Supportive Housing staff, Department of Human Services, and the Department of Public Health.
  • Monitors the effectiveness of the Jelani House programs and develops procedures to enhance services. Ensures contractual obligations are met, reports are completed on time, and monitors outcomes with an eye to continuous improvement.
  • Develops and implements performance standards to ensure adherence to Intensive Case Management service model and agency policies. Ensure that required client documentation is captured by staff in the data reporting system and in client files. Engage in quality improvement practices including data collection and analysis, and monitoring of program outcomes.
  • Assists in the development of program budgets, program goals and objectives to deliver quality services to clients. Monitors budget spending.
  • Participates in any scheduled contract evaluations.
  • Attends weekly Management Team meeting to address program issues.
  • As a member of HPP’s management team, support the implementation of our Theory of Change and mission alignment initiative, internally known as Moving our Mission Forward. Launched in 2017, we are now in the implementation stage and are actively learning from our program model how HPP can best focus on its core mission to support homeless families, and families at greatest risk of being homeless, with children zero to five years with a goal of more effectively achieving powerful and transformative changes in their lives.
  • Oversees the development and maintenance of relationships with other providers that support the agency’s mission and programs.
  • Represent HPP in the community.

Qualifications:

  • MSW or Master’s Degree in a related field preferred. LCSW strongly preferred.
  • At least two years case management experience.
  • At least five years of supervisory experience.
  • Experience in a residential setting, strongly preferred.
  • Experience and knowledge of maternal health in particular prenatal and postpartum.
  • Knowledge and understanding of the Bayview/Hunter’s Point community required and Bayview community members strongly encouraged to apply.
  • Understanding of, and commitment to, social justice, and HPP’s mission and organizational values.
  • Demonstrated knowledge of pregnancy and birth and maternal and child health.
  • Knowledgeable about the dynamics of homelessness, families, substance abuse, mental health, domestic violence, and parenting.Other preferred qualifications:

Physical Requirements: This position requires the ability to work under stress and handle multiple project deadlines. This position requires an annual tuberculosis test and may require moderate lifting.
