Director of Social Services

Beautiful 5 Star Healthcare facility looking for a qualified social worker to join our team.
ONLY APPLICANTS WITH CSW, LSW OR ABOVE WILL BE CONSIDERED FOR THE POSITION. Prior nursing home experience preferred.

Responsibilities include:

• planning and implementing a comprehensive social services program that offers counseling and other support services for residents and their families
• evaluating and being keenly aware of residents’ medical-related social needs and then formulating the appropriate plan for treatment that fits into their total care plan
• coordinating and implementing family meetings for residents and creating a forum for ongoing discussions between residents, their families/responsible parties and the treatment team
• maintaining residents’ rights at all times while liaising between all parties involved
• coordinating discharge planning and assisting in the creation of an organized discharge plan for all residents
• and above all, ensuring that residents and their families receive the highest quality of service in a caring and compassionate atmosphere that recognizes the individuals’ needs and rights
