CES Case Manager
Location: Paterson NJ
Hours: Part Time- Non-Exempt; 20-25 hours per week; Monday — Thursday; 9:00am-2:00pm

Job Summary:
The goal of the program is to foster economic self-sufficiency for families and individuals by providing financial assistance and case planning. The program is designed to develop comprehensive counseling services and educate the public concerning housing opportunities, while assisting to navigating the resources available. The program is a collaborative partner under the Passaic County Continuum of Care committed to addressing the needs of homeless or potentially homeless families in Passaic County. The case manager functions as a conduit to process assistance applications and ensure financial assistance is rendered. The case manager is responsible for the coordination, development and planning goals to promote self-sufficiency.

Duties and Responsibilities:
l . Case managers maybe assigned walk-in duties for CES programs, in addition to caseload.
2. Meet face to face with clients. Determine client needs based on interview, develop a budget, develop Housing Stabilization Plan, provide housing stabilization services intended to meet basic essential needs; including: advocacy and referral; Assessing eligibility for entitlements/financial assistance.
3. Work with clients to identify long-term solutions to recurring problems. Network with parishes, community/government organizations to effect maximum utilization of available resources to meet basic essential needs.
4. Develop and maintain accurate case management plan, case files and monthly statistical reports. Enter all client information into AWARDS.
5. Provide follow-up services as needed. Including housing inspection checklist prior to providing financial assistance.
6. Work independent with accuracy and complete adherence to policy and procedures.

Skill and Requirements:
1. College degree or equivalent work experience.
2. Minimum 1 year direct case experience..
3. Ability to work with a cross section of the population including homeless families.
4. Strong organizational skills
5. Ability to serve as a liaison between consumer and third parties.
