Additional focus and responsibilities:
Responsible for leadership and oversight of the management of daily operations and budget of Utilization Management, Inpatient Care Coordination, and Outpatient Care Management.
Responsible for creating and supporting processes that support communication and care across the continuum that improve clinical efficiencies, throughput of the hospital, and actively support of patientâs comprehensive needs including social influences of health and patient navigation across the HMC continuum.
Responsible for key metrics including length of stay, readmissions, authorization related financial performance and patient activation measure in the community, ensuring stakeholder collaboration.
The successful candidate must have a minimum of ten years care management/utilization management and/or related experience within a hospital and/or ambulatory setting along with seven years progressive leadership experience in healthcare.
MINIMUM QUALIFICATIONS:
Bachelor’s Degree in Nursing required.
Master’s Degree in Health related or Business field required.
Current RN licensure required.

Ability to analyze data.
Project management skills.
Senior leadership experience.
Process improvement skills.
Ability to lead and facilitate work towards organizational goals.
Excellent verbal and written communication skills.

PREFERRED QUALIFICATIONS:
Certification in Case Management is strongly preferred.
