This is an exciting opportunity to oversee the Care Transitions functions across the health care continuum of the Penn State Hershey Medical Center and the Academic Practice Division of the Medical Group in collaboration with associated partners across the system, and community organizations. This work includes the integration of Acute Care Social Work and Care Coordination, Utilization Management, Outpatient Adult Care Management, and Outpatient Pediatric Care Management. In addition to daily oversight of operations, this person will partner with Revenue Cycle and Clinical leadership to create patient centric processes and interdisciplinary relationships that result in seamless patient care.
The Vice President of Care Transitions is the senior leader over care transitions for MSHMC Academic Practice Divison patients that includes coordination of throughput to the hospital and ambulatory practices, informed decision support on authorization approvals for patient encounters, and case management for patients that transition to the community to enhance outcomes and prevent readmissions. The VP is also responsible for building relationships and creating partnerships with community organizations and Penn State Health quality partners.
This VP, Care Transitions reports directly to the Senior Vice President/Chief Medical Officer in a triad with the Senior Vice President/Chief Nursing Officer and the Vice President Revenue Cycle. There are five direct reports: Manager Care Coordination, Manager Social Work, Manager Utilization Management, Manager Outpatient Care Management and Manager Pediatric Care Management with approximately 200 indirect reports.

Additional focus and responsibilities:

Responsible for leadership and oversight of the management of daily operations and budget of Utilization Management, Inpatient Care Coordination, and Outpatient Care Management.

Responsible for creating and supporting processes that support communication and care across the continuum that improve clinical efficiencies, throughput of the hospital, and actively support of patient’s comprehensive needs including social influences of health and patient navigation across the HMC continuum.

Responsible for key metrics including length of stay, readmissions, authorization related financial performance and patient activation measure in the community, ensuring stakeholder collaboration.

The successful candidate must have a minimum of ten years care management/utilization management and/or related experience within a hospital and/or ambulatory setting along with seven years progressive leadership experience in healthcare.


MINIMUM QUALIFICATIONS:

Bachelor’s Degree in Nursing required.

Master’s Degree in Health related or Business field required.

Current RN licensure required.

Senior leadership experience.

Process improvement skills.

Ability to lead and facilitate work towards organizational goals.

Excellent verbal and written communication skills.

Evidence of successful inter-professional collaboration.

PREFERRED QUALIFICATIONS:

Certification in Case Management is strongly preferred.
