The Case Manager will report directly to the Executive Director and will be responsible for the following duties:
a. Complete all client intake paperwork and work with Clinical Services Director to complete intake risk and safety assessments;
b. Accessing client needs, and developing strategies to help the client;
c. Ensure that service delivery is performed according to the Annunciation Homeâs mission, policies and procedures, and service philosophy;
d. Provide support and resources for clients; good working relationship with support networks, government resources and community resources;
e. Researching and referring client resources such as healthcare, childcare, educational and employment opportunities;
f. Working with Client Care Team to respond to crisis situations that affect client and their children;
g. Evaluating and studying programs, services and resources for quality and client needs;
h. Working with the Client Care Team to determine correct course of action when helping client and child;
i. Develop, maintain, and sign-off on service plans for clients, which includes but is not limited to scheduling for medical appointments and transportation. In addition to the development of short- and long-term goals in the areas of education, employment, and family relationships.
j. Hold weekly sessions with each client individually to address decision making, planning, and overall well-being and document meeting date and time;
k. Participate in weekly Client Care Team Meetings with the Executive Director and other team members in order to review each clientâs progress through her service program;
l. Coordinate parent conferences and work with parents to improve and maintain their relationship with their daughter and to work with her to plan for her future;
m. Coordinate conferences with the birthfather and extended family of the baby (when appropriate) to help them and the birthmother work through their feelings/plans;
n. Work with Clinical Services Director to assist clients with problems that affect her ability to have satisfactory personal relationships and to develop her ability and capacity for growth;
o. Be responsible for career counseling and referral to other agencies when appropriate;
p. Complete discharge and/or transfer summaries and finalize financial support for each birthmother by coordinating with respective social service agencies for assistance;
