Responsibilities
- Assesses needs and conducts comprehensive and ongoing assessments. Monitors adherence to treatment plans and service needs.
- Understand the needs of the patient by scrutinizing their background history, understanding their current needs, and arranging for their well-being,
- Coordinate with various service providers to ensure the fulfillment of the patient needs,
- Ensure proper follow ups, and have face-to-face interactions with the patient, and other parties involved,
- Make it feasible for the patient to access increased supply of services as per requirement,
- Possess adequate knowledge about the availability of resources, and the desired paperwork that needs to be done to ensure successful resolution,
- Maintain confidentiality regarding information shared by the patient,
- Keep proper records of all the work performed with the patient. This involves records of the patientâs information, the meetings held with the patient and service provider(s), details of the referrals that were issued, the plans and goals that were outlined to ensure a positive outcome.
- Maintain relationship with patient until services are no longer needed.
- Must be able to apply principles of critical thinking to a variety of practical and emergent situations and accurately follow standardized procedures that may call for deviations.
- Must be able to apply sound judgment beyond a specific set of instructions and apply knowledge to different factual situations.
- Must be alert at all times; pay close attention to details.
- Must be able to work under stress on a regular or continuous basis.
Job Type: Full-time
Salary: $15.00 to $17.00 /hour
