JOB SUMMARY: The Behavioral Health Director is responsible for facilitating care for patients in need of enhanced services for behavioral health. Their role is to improve health outcomes through coordinating care, educating patients, conducting brief behavioral health interventions, building trust between patients and medical and behavioral health practitioners, and enhancing communication and integration of care. Eligible patients will receive timely, high quality and efficient health care and support services through the development of a multi- disciplinary care plan, self-management goals, and referrals to both internal and external resources. The Behavioral Health Director role is key to creating a patient centered environment for all patients.
ORGANIZATIONAL DUTIES AND RESPONSIBILITIES

ESSENTIAL JOB FUNCTIONS:
- Identify and link patients with medical and behavioral health needs.
- Link patient’s community resources to facilitate and respond to social service needs.
- Track and support patients when they obtain services outside the practice, i.e. Emergency Departments,
Hospitals, and other behavioral health and medical health care facilities.
- Work in collaboration and with practitioners, communicate test results and care plans to patients/families.
- Provide relevant self-management support for patients with chronic illnesses as identified by clinical teams.
- Work collaboratively with other team members to provide care management services for high risk patients.
- Monitor lifestyle factors affecting health, i.e. tobacco use, substance abuse, nutrition and physical activity, and assist the patient with goal setting to achieve behavioral change.
- Collaborate with other members of the team for care coordination and care management activities, including implementation of Care Plan.
- Manage referrals, when needed, to appropriate agencies required to assist the client in achieving the goals and objective identified in their Care Plan.
- Identifies significant changes or high-risk situations in patient’s condition and takes appropriate actions as evidenced in reporting and documentation.
- Provides triage for urgent patient calls.
- Provides patient/patient’s family with appropriate education based on identified needs to optimize patient/patient’s family understanding and documents response according to standard operating procedure.
- Completes all appropriate charting and documentation.
- Notifies Medical Director of any complaints, unsatisfactory outcomes or special patient needs or requests.
- Schedules referral appointments and obtains all necessary information for the patient.
- Adjusts own schedule to meet fluctuation in workload.
- Understand and effectively communicate HIPAA compliance, corporate compliance, and client confidentiality.
- Ensure and/or remain in compliance with local, state, and federal regulations.
- Supervise Behavioral Health Interns and other staff relating to patient care coordination (i.e. Social
Services Care Coordinator).
- Complete other duties as assigned.
JOB QUALIFICATIONS:
Required:
- Master’s Degree in a related field
- Experience in behavioral health and case management
- Strong written and verbal communication skills
- Teamwork orientation
- Familiarity with local community resources
- Able to take and follow through with delegated tasks and accountability
- Proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint
- Ability to demonstrate excellent customer service and strong cultural competency
- Ability to adhere to strict confidentiality guidelines
Preferred:
- Experience with the Health Resources and Services Administration (HRSA)
- One to two year’s clinical experience, preferably in a clinic setting, managed care, community health, or related field
- Prior experience with care coordination/care management in a medical setting. Prior experience with managing competing priorities
- Prior experience working with diverse population groups
- Experience with program development
WORKING CONDITIONS:
- Work is performed in a clinical setting and requires some hands-on patient care.
- OSHA category: High risk. Job requires performance of tasks that involve exposure to blood, body fluids or tissue.
PHYSICAL REQUIREMENTS: The Behavioral Health Director is primarily a desk job that requires the ability to sit for long periods of time (up to 8 hours) at a desk or workstation. Periodically, the position requires lifting or carrying items that would be appropriate in an office environment, not more than 20lbs.
