DIRECTOR OF CARE COORDINATION
(Administrative Manager III Specialty)

Salary may be negotiable, based on position requirements and successful candidate’s qualifications, subject to appropriate authorization.

This position is scheduled to receive salary range increases on the following dates:

Effective July 3, 2020 – 2.5% increase
Effective July 2, 2021 – 2.5% increase
Effective July 1, 2022 – 3.0% increase

Mandatory Employee Retirement Contributions will decrease on the following dates:

Effective July 3, 2020 – by 1.2% decrease
Effective July 2, 2021 – by 1.2% decrease
Effective July 1, 2022 – by 0.5% decrease

The County of Orange Health Care Agency (HCA) is a dynamic organization dedicated to creating and supporting an environment that promotes the achievement of optimal individual, family and community health. Under the direction of the Agency Director, a new and dynamic executive team with an organizational structure comprised of six service areas – Administrative and Financial, Behavioral Health, Care Coordination, Correctional Health, Regulatory & Medical Services, and Public Health – is committed to meeting the health needs of the diverse Orange County community.

THE OPPORTUNITY

HCA seeks an experienced and proven leader to manage the newly established Care Coordination Service Area. The successful candidate will be a self-starter who will oversee the development of a collaborative system of care that enhances access to services including shelter services, mental health services, health care, community corrections, as well as benefits and supportive services. This individual will advise the Board of Supervisors, County Executive Office and the HCA Executive Team of ongoing issues and outcomes as well coordinate with public, private and community stakeholders.


THE POSITION

Reporting directly to the Director of HCA, the position will be responsible for developing an integrated system of care, improving quality and efficiency of care and the delivery of services; making policy recommendations on funding and service delivery models; working with public, private and non-profit partners to support and collaborate on a strategic plan for an integrated system of care; and overseeing the Care Coordination and Shelter Services units.
The Director of Care Coordination will:
  • Demonstrate experience managing a medium to large team tasked with developing and implementing new programs
  • Demonstrate extensive knowledge in homelessness, housing, child welfare, and disability programs
  • Possess significant experience managing and implementing technology solutions for large community based projects
  • Demonstrate expertise in population based strategies to address the needs of youth, young adults, families, and single adult homelessness
  • Manage and monitor funding, Memorandums of Understanding and contracts among County Departments and community providers
  • Identify and implement strategic, short and long range planning and programmatic framework to achieve goals
  • Act as a liaison to and build strong working relationships with County officials and departments, community organizations, and other non-profits regarding issues of and relating to homelessness
  • Develop qualitative and quantitative measures to monitor, evaluate programs and projects, contract and grant compliance, and customer satisfaction
  • Prepare and recommend project and program budgets and participates in preparation of budgets that support services
  • Demonstrate political savviness when responding to situations requiring tact, diplomacy and persuasion


DESIRABLE QUALIFICATIONS

The ideal candidate will possess a bachelor’s degree or higher in Public Administration, Social Sciences, Business Administration, or a related field, and over seven (7) years of experience in public administration, including five (5) years of experience in the design and coordination of large community based projects using technology solutions. Experience managing and supervising in a large, complex organization is highly desirable.

In addition to the above, the ideal candidate will demonstrate the following key competencies:


Systems of Care Expertise:
Demonstrated experience designing, developing, and delivering systems of care such as Housing, Healthcare and/or Behavioral Health. Hands-on experience applying methods of financing of care coordination projects, and administering the principles and practices of homeless services strategies.

Management Skills:
Demonstrated experience managing a medium to large team; managing programs within budgetary guidelines; designing, developing and implementing program objectives and procedures; ensuring compliance with policies, regulations, and laws; articulating a vision for future program goals and setting expectations for staff; conducting strategic planning to review and revise program objectives as needed; identifying innovative solutions and acting decisively; and reviewing Board memos, requests for information through the Public Records Acts (PRA), press releases, court documents and operational plans.

Project Management:
Demonstrated experience managing large projects that use technology solutions to develop, execute and monitor project; identifying resources and developing budget; identifying risks or issues and recommending solutions, establishing timelines and milestones for project, delegating tasks to team and using project management tools and techniques such as SharePoint and/or Microsoft Office to track status.

Finance & Grant Administration Expertise:
Demonstrated experience with the principles, practices and techniques of research and statistical analysis, grant writing and administration, financial forecasting, budget administration, and working knowledge of federal, state and local policies relating to homelessness and financing.

Communication:
Outstanding skills in both spoken and written communications including the ability to write cogently and concisely, with excellent grammar, spelling and punctuation; Demonstrated ability developing and making presentations effectively to audiences at all levels with tact, diplomacy and persuasion. Superior skills in engaging audiences, explaining complex processes and demonstrating active listening.

Collaboration:
Demonstrated ability working effectively with others; establishing and maintaining strong working relationships with public, private, and community stakeholders, Elected Officials and Government Officials at all levels of government; and working with various groups in creating a forward-thinking approach in developing an integrated system of care.

Critical Thinking:
Demonstrated ability to interpret data, develop conclusions, and develop and implement action plans that resolve issues. Experience in solving complex community concerns and developing effective solutions.
