Job Summary Social Services Director
The primary purpose of your position is to plan, organize and implement the overall operation of the Facilityâs Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator. You are to assure that the medically related emotional and social needs of the resident are met and maintained on an individual basis.
Responsibilities and Duties of Social Worker
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Interview residents and families to obtain social history.
- Involve the residents and families in planning social service programs when possible.
- Refer residents and families to appropriate social service agencies when the Facility does not provide the services or needs of the resident.
- Provide information to residents and families as to Medicare and Medicaid, and other financial assistance programs available to the resident.
- Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.
- Maintain a quality working relationship with the medical profession and other health related facilities and organizations.
- Coordinate social service activities with other departments, as necessary.
- Assist in providing solutions for social and practical environmental problems including seeking financial assistance, discharge planning (including collaboration with community agencies), and referrals to other community agencies when specialized assistance is required.
- Evaluate social and family information and assist in determining plans for social treatment.
- Interpret social, psychological, and emotional needs of the resident and family to the medical staff, attending physician, and other resident care team members.
- Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed.
- Ensure that all charted progress notes are informative and descriptive of the services provided and of the residentâs response to the service.
- Involve the resident and family in planning objectives and goals for the resident.
Qualifications and Skills for Social Worker
Must possess, as a minimum, a Bachelorâs Degree in social work, sociology, special education, rehabilitation counseling, psychology, or any other specialized intellectual instruction directly related to social work. Must have SNF experience
