Job Purpose:

Reporting to the Chief Executive Officer (CEO), the project manager oversees activities related to the collaboration initiative between El Buen (EB) and Lone Star Circle of Care (LSCC). The position will serve as the liaison between El Buen and Mission Capital (MC) and will monitor contract established with MC for purpose of facilitating the planning proposal between EB and LSCC. Works with internal and external partners/consultants to ensure that the established planning year phases and goals are accomplished; collaborates with program and executive leadership to ensure appropriate coordination between the EB, MC, and LSCC; maintains a project management system to identify key activities status, challenges, solutions, and ongoing reporting; collaborates with consultants to coordinate internal and external stakeholder meetings, identifies and/or monitors success benchmarks and measurements related collaboration with LSCC, develops collaborative community relationships with multiple and diverse partners in the public health, health care, social services, and academe; provides leadership and administrative oversight for area of responsibility; participates in grant writing to ensure the sustainability of program area, supervise staff as assigned or the work of staff/teams not directly reporting up to the position. Note: This is a temporary full-time position with the potential to become permanent positions after the first year.


Essential Functions:


  • Development, Implementation and Evaluation of Health & Social Services Integration Programs

    • o Serves as the project manager for the EB-LSCC planning initiative.
    • o Oversees and monitors the various phases and goals of contracts with partners and contractors – this includes managing the contract with MC to include planning, strategy formulation, beta testing, implementation, monitoring, and evaluation
    • o Supports the CEO in developing successful benchmarks; deploys strategies and tactics to achieve goals
    • o Supports organization’s efforts to develop and maintain external partnerships and collaborations
    • o Conducts research and analysis on sector- accepted best practices to include organizational collaborations and best practices, value-based metrics, performance measures, data collection, evaluation, social determinants of health

  • Project Management and Support

    • o Establishes a project management system to address timelines, challenges, solutions and facilitate the ongoing reporting to leadership and funder(s).
    • o Provides staff support to the CEO for the convening of a range of meetings including the El Buen-LSCC advisory committee meetings focusing on short-or long-term projects related to the collaboration with LSCC. This may include undertaking research, tracking trends, preparing presentations; development of meeting agendas; compiling pertinent background materials for attendees; preparing summaries of discussion; coordinating logistical arrangements; and ensuring appropriate and effective communications with participants, as needed.
    • o Participates as part of the senior leadership team

  • Human Resources

    • This position is responsible for overseeing the work of multiple and diverse staff that do not directly report up to the position. Communicates frequently, honestly and openly with staff and contractors and provides clear direction and priorities; promotes teamwork and inclusiveness.

  • Fiduciary Responsibilities

    • Manages allocated budget, expenditures, and negotiates updates as needed

  • Grant Writing and Contracts


    • Assists with the research, development, and writing of grant proposals
    • As needed, prepares data required for the submission or reports for funders or donors as requested
  • Other Duties
    • Performs other related duties as assigned


Minimum Qualifications:

  • A bachelor’s degree from an accredited institution in public health, health education, community health, nonprofit management, social work or related field; Master’s degree strongly preferred
  • A minimum of five (5) years of proven work experience in the health care and/or public health or related field with at least two (2) years full-time experience directly related to program development, project management and/or staff supervision. Prefer knowledge/understanding of medical billing and reimbursement
  • Demonstrated track record of thinking conceptually and mastering complex subject matter quickly.
  • Demonstrated capacity to develop and implement strategies, plans, and solutions to challenges and problems.
  • Proven ability to develop strong community partnerships at multiple levels
  • Proven ability to effectively communicate (written and oral) and work well with a variety of stakeholders from different socio-economic and cultural backgrounds
  • Experience implementing culturally relevant services/projects
  • Experience developing or writing funding proposals/grants
  • Proficiency in Microsoft Office suite (Word, Excel, Outlook, PowerPoint)
  • An equivalent combination of education and experience sufficient to perform the essential duties of the position
  • Bilingual English/Spanish strongly preferred
  • Strong project management experience preferred


Work Environment and Physical Demands:

  • Continuous indoor work; occasional outdoor work
  • No hazardous or unpleasant conditions caused by exposure to extreme temperatures, machinery, noise, dust, etc.

  • Ability to work independently and to undertake project/staff supervisory responsibilities as needed.

  • Must have a valid TX driver’s license with a good driving record; insurable by El Buen’s auto policy

Salary: $58K – 68K (Salary to commensurate with experience)
