PROGRAM DESCRIPTION:
For many years, BRC has provided home-based case management services through its Blended Case Management BCM activities. With Medicaid reform, BCM has transitioned into BRC’s Care Coordination program. Funded by Medicaid and the NYC Department of Health and Mental Hygiene and Medicaid, BRC Care Coordination offers comprehensive case management services to individuals diagnosed with a serious and persistent mental illness, many with a history of homelessness and/or substance abuse. BRC maintains an ongoing enrollment of over 100 clients in its Care Coordination program.
HOURS:
Full-time, 35 hours per week
Monday – Friday, 8:30am – 5:00pm
DUTIES/RESPONSIBILITIES:
Supervise clinical staff in the provision of services; recruit new employees; train staff on day to day program operations and protocols; work in conjunction with the LPN to ensure medication compliance; conduct monthly chart reviews to keep in line with NYSOMH regulations and standards; run groups as needed; participate and/or stand in for the Program Director in critical incident review meetings; complete various monthly reports; attend case conferences; manage crises; network with New York City’s Supportive Housing facilities for housing referrals.; provide 24 hour emergency coverage. Related duties as assigned.
QUALIFICATIONS:
BA required. Experience with MICA population preferred. Supervisory experience preferred. Knowledge of PACT WEB/HRA 2010E program and Microsoft Office Suites required. Knowledge of AWARDS client based practice management system helpful. NYS Driver’s License required. Ability to drive a thirteen passenger van if needed.
