Westmoreland Rehab is a 66 bed Skilled Nursing Facility in Colonial Beach, VA.

As the Director of Admissions you will be responsible for the following:

  • planning, directing and monitoring the daily operations of the Admissions Department
  • Ensure budgeted resident census goals are met and that residents are admitted in a professional and efficient manner.
  • Facilitating admissions from assigned hospitals on a daily basis
  • Establishing and maintaining positive relationships with hospital Case Managers, Social Workers, Discharge Planners, Insurance Case Managers and Physicians.
  • Conducts on-site evaluations of hospital patients to assess suitability for facility placement
  • Visits assigned referral sources daily to provide information regarding facility services and bed availability, assess patients for potential admission, and develop / enhance professional relationship with hospital discharge planners and other key referral individuals.
  • Maintains comprehensive knowledge of current Medicare and Medicaid guidelines pertaining to reimbursement for skilled care in Virginia.
  • Plans and delivers effective presentations to external referral source groups or individuals in order to introduce and explain facility services and/or provide clinical education.
  • Personally visits all facility residents who have been hospitalized locally to demonstrate concern, connect with family if applicable, and assess potential for facility re-admission.
  • Seeks involvement in community groups, meetings, events, etc. that may lead to enhanced awareness of facility services and increased referrals.
  • Assists in planning and execution of outreach events such as on-site facility health fairs, educational programs, and other promotional events designed to attract hospital discharge planners, physicians, and other key persons to the facility.
  • Completes additional facility-specific ongoing tasks and projects as assigned by immediate supervisor

80-90% in state travel

QUALIFICATIONS

  • Bachelor’s Degree in health care or related field required.
  • Two years Admissions experience in long term care.
  • Must have valid Driver’s License and access to transportation as needed for frequent visits to external referral sources.
