Job Description

The Transition Project Manager is directly responsible for the successful transition of the operations services and technical tools for the Medicaid fiscal agent framework from the current contract to the new task order. The Transition Manager is responsible for the efficient transition of technical and programmatic support functions, the successful transition of governance processes, and delivering outcomes that result in the government achieving a successful contract transition. Manage and integrate transition activities in accordance with contract requirements and ensure technical, schedule, cost, and financial performance. Manage transition risk, customer relationships, and transition staffing.

remotely near any US Conduent location with travel***


Roles and Responsibilities

  • Coordinate and support weekly status meetings discussing weekly transition activities with key customer stakeholders.

  • Ensure a transparent and seamless transition maintaining existing performance levels

  • Maintain support for ongoing projects

  • Follow customer’s Risk Management Policy

  • Minimize transition costs

  • Requires strong written and verbal presentation skills including formal and informal presentation of work and assessments to critica
    l audience.


Skills and Qualifications


  • Minimum of five (5) years’ in an IT Project Management position

  • PMP certification

  • Excellent written and presentation communication skills

  • Education:
    Bachelors and five (5) plus years or more experience.
