Harmony Hospice has an immediate opening for a compassionate and caring CLINICAL DIRECTOR/DON for our Cleveland Ohio/Cuyahoga County area.

Essential Job Responsibilities:

  • Responsible for the administration, planning, provision, and evaluation of all services.
  • Supervise all direct patient/family care team members, including nurses, social workers, home health aides and counselors. Also, participate in the hiring and evaluation process of clinical team members.
  • Coordinate the development, implementation and review of the interdisciplinary plan of care for each patient/family.
  • Attends the interdisciplinary group meeting on a regular basis and serves as a liaison with the Medical Director and clinical staff. Conduct the interdisciplinary team meetings, maintains appropriate documentation, or designates these functions to a qualified team member.
  • Participates/oversees in ongoing quality assurance activities related to patient and family services, including QI studies and chart audits to ensure the quality of patient/family services.
  • Ensures that all patient and family services offered by the Hospice comply with federal, state(s), and local regulatory requirements.
  • Review admission data and prognostic indicators to assure appropriateness of hospice patients on service.
  • Promotes/assists staff in self-improvement.
  • Orientation of all new employees followed by evaluation of skills.

Desired skills and experience:

  • Minimum three years’ experience as a Case Manager or 1 to 2 years as an Assistant Director of Nursing/Clinical Coordinator/ Director of Nursing.
  • Must have a reliable vehicle, clean driving record, valid license.
  • Valid RN license, State of Ohio.
  • Bachelor’s degree in Nursing required.
  • Be available at all times, either in person or by telephone.
  • History of managing employees in a clinical setting.

Job Requirements:

  • Must have a current OH, RN license
  • Must have a reliable vehicle, clean driving record, and valid driver’s license.
  • Must have a minimum of 3 years of Case Manager experience.
