Harmony Hospice has an immediate opening for a compassionate and caring CLINICAL DIRECTOR/DON for our Cleveland Ohio/Cuyahoga County area.
Essential Job Responsibilities:
- Responsible for the administration, planning, provision, and evaluation of all services.
- Supervise all direct patient/family care team members, including nurses, social workers, home health aides and counselors. Also, participate in the hiring and evaluation process of clinical team members.
- Coordinate the development, implementation and review of the interdisciplinary plan of care for each patient/family.
- Attends the interdisciplinary group meeting on a regular basis and serves as a liaison with the Medical Director and clinical staff. Conduct the interdisciplinary team meetings, maintains appropriate documentation, or designates these functions to a qualified team member.
- Participates/oversees in ongoing quality assurance activities related to patient and family services, including QI studies and chart audits to ensure the quality of patient/family services.
- Ensures that all patient and family services offered by the Hospice comply with federal, state(s), and local regulatory requirements.
- Review admission data and prognostic indicators to assure appropriateness of hospice patients on service.
- Promotes/assists staff in self-improvement.
- Orientation of all new employees followed by evaluation of skills.
Desired skills and experience:
- Minimum three years’ experience as a Case Manager or 1 to 2 years as an Assistant Director of Nursing/Clinical Coordinator/ Director of Nursing.
- Must have a reliable vehicle, clean driving record, valid license.
- Valid RN license, State of Ohio.
- Bachelor’s degree in Nursing required.
- Be available at all times, either in person or by telephone.
- History of managing employees in a clinical setting.

- Must have a minimum of 3 years of Case Manager experience.
