As Director of Resident and Family Services I & II, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Responsible for providing services to respond to the emotional needs of the residents and their families. You will also assist with resident admissions and the referral process. No direct supervisory function. This job description does not list all the duties of the job. You may be asked by the supervisors or managers to perform other duties. Duties and Responsibilities

1. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Executive Director.

2. Implement policies and procedures for the identification of medically related social and emotional needs of the resident.

3. Participate in community planning related to the interests of the facility and the services and needs of the resident and family.

4. Participate in discharge planning; development and implementation of social care plans; and resident assessments.

5. Involve the resident/family in planning social service programs when possible.

6. Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs.

7. Maintain a quality working relationship with the medical profession and other health related facilities and organizations.

8. Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of such to the Executive Director.

9. Maintain a reference library of written material, laws, standards of practice, etc., necessary for complying with current standards and regulations and that will provide assistance in maintaining quality social service.

10. As part of the admission process, meet with the resident and family to review policies on advances directives, resident rights, and non-discrimination.

11. Conduct and document a social services evaluation, including identification of resident problems/needs.

12. Provide/arrange for social work services as indicated by resident/family needs.

13. Assist residents, as needed, with financial planning.

14. Maintain communications with caseworkers.

15. Keep the resident’s family informed of resident problems, personal needs, transfers, and changes of level assignment.

16. Maintain a current list of community resources to facilitate referrals.

17. Meet with the appropriate facility staff concerning resident issues.

18. Act as a liaison between the facility and the community.

19. Ensure all documentation complies with applicable regulations.

20. Act in compliance with all corporate, state, federal, and other regulatory standards.

21. Provide social work consultation to residents, families, and staff, as required.

22. Participate in and/or provide inservice education sessions.

23. Participate in the quality improvement process of the facility.

24. Access continuing education opportunities appropriate to discipline and responsibilities.

Resident Rights

Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.

Knock before entering a resident’s room.

Ensure that all social services personnel are knowledgeable of the resident’s rights and responsibilities including the right of refusal.

Review complaints and grievances made by the resident and make a written/oral report to the Executive Director indicating what action(s) were taken to resolve the complaint or grievance. Follow facility’s established procedures.

Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.

Participate in resident/group council meetings as requested and provide support services to such council.

Working Conditions

Works in office areas as well as throughout the facility.

Moves intermittently during working hours.

Is subject to frequent interruptions.

Is involved with residents, personnel, visitors, government agencies/personnel, etc., under all conditions and circumstances.

Is subject to hostile and emotionally upset residents, family members, etc.

Communicates with the medical staff, nursing service, and other department directors.

Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary.

Is subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).

Attends and participates in continuing educational programs.

Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants.

Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and AIDS and Hepatitis B viruses.

Maintains a liaison with other department supervisors to adequately plan for social services/activities.

May be subject to the handling of and exposure to hazardous chemicals.

Education

Must possess, as a minimum, a Bachelors Degree, LSW or MSW from an approved school of Social Work. Current license/certification in the state of practice.

Experience

Minimum of one (1) year experience in long-term care preferred.

Demonstrated ability to assess resident needs, develop, and implement a plan of care and coordinate the discharge planning process.

Must possess knowledge of accreditation standard and compliance requirements.
