The Project Manager for the Community Resource Connections Program, an initiative designed to streamline and improve institutional efforts to address social determinants of health (SDoH) needs in vulnerable patient populations, will be responsible for managing assigned projects from start to finish, including all phases and resources utilizing industry standard project management tools and techniques.

• Responsibilities will include managing and supporting various program development and implementation strategies, data collection and analysis, complex case management with vulnerable patient populations, preparing deliverables, meeting coordination, and administrative tasks as necessary.
• This role will also be a member of the project leadership in partnership with key stakeholders.

• Work with all key stakeholders to manage the deployment and completion of all aspects and stages of the project assigned.
• Ensure requirements are managed and delivered per project timeline, as well as to Identify and mitigate potential risks for implementation.
• Partner with leadership to assess, track and resolve.
• Develop and document plans that ensure all necessary preparation, execution tasks and dependencies are identified and included.
• Ensure that necessary resources and skills are allocated for project success.
• Assess needs and escalate, if needed.
• Responsible for the oversight of functional project team, including, partnering closely with subject matter experts to help gather, assess and track project deliverables.
• Hold project teams accountable for project deliverables.

Project management & collaboration:

• Proactively manage projects by using critical project management thinking skills- including but not limited to establishing scope of work and project plan, managing timelines for project tasks, reporting, ensuring goals are tracked and met, and effectively coordinating activities with project team.
• Provides cross-functional solution direction and guidance, including aiding in resolving any conflicting project decisions. Discuss business case scenarios, present to upper management and guide team on implementing the best solution through consensus building.
• Operates as a full cross-functional leader/member of the project team; supporting project leadership and other team members as directed.
• Complete projects independently or with appropriate staff as identified by the Supervisor or Director.
• Regularly interfaces with institutional executive leadership to provide updates on project tasks, implementation, and success.
• Facilitates taskforce initiatives and spearheads focus groups.
• Identify, facilitate, and foster collaborative partnerships with community-based organizations to build referral network to meet social needs of vulnerable patient populations.
• Participate in process of identifying service gaps and opportunities for targeted grant-making related to vulnerable patient populations.
• Works collaboratively and professionally with clinical staff including social workers and community health workers when necessary to mange needs of complex patient populations.
• Assist with the identification, retrieval, manipulation, and analysis of data necessary to make decisions to support strategic initiatives, track project metrics, measures of success, and ongoing program management activities.
• Assist management and executive staff with other special projects and functions as necessary.
• Coordinate meetings between SDoH management team, steering committee, stakeholders, and community-based providers.
