As a Director of Quality Improvement, you will provide leadership, oversight and strategic direction to a coordinated Population Health Management care model that spans the continuum of care designed to improve quality, reduce cost, and promote patient and provider satisfaction. You will be responsible for partnering with key physician and practice stakeholders in setting the strategy for an effective continuum-based care model including but not limited to: Transitions of Care, Chronic Care Management, Remote Patient Monitoring and Post Discharge Home Visits for an accountable care organization (ACO). Time will be split 50/50 between team management and direct patient care, during Post Acute Home Evaluations.

Ability to manage an interdisciplinary team composed of medical assistants, paramedics, RN and pharmacist. Provide reports on ACO programs mentioned above to 15 primary care practices on a monthly basis. Contract negotiation of above-mentioned programs with primary care practices. Conduct at risk reports for 59 offices on a quarterly basis. Ability to complete implementation plans for CMS benefit enhancements. Public speaking and presenting skills for FLAACOS and NAACOS conferences.

Position reports directly to the ACO CEO. Our programs are focused on ensuring that patients, especially the chronically ill patient population receive the right care at the right time at the right level, while avoiding unnecessary duplication of services and preventing avoidable hospital admissions.

Qualifications:

Strong Clinical Skills and Ability to Work Independently on Multiple Projects

3 Years Management, Direct and Remote Interdisciplinary Team Leader, Case Management, Supervisory and Teaching Experience Preferred

Ability to Conduct Presentations and Public Speaking, Implement Innovative CMS Model Programs, Assign/Develop Workflows, Train and Mentor Personnel Strongly Preferred

ARNP License Required

Bilingual English/Spanish Preferred
