Position Summary:
The Case Manager will provide case management, assessment, and treatment planning services to patients with a Substance Use Disorder. Case management services mean those activities provided to assist and support individuals in gaining access to needed medical, social, educational and other services essential to meeting basic human needs. Case management services may include interactions with family members, other individuals or entities, and includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, and linkage to appropriate services. Case management services at BrightView consist of: (1) assessment of the eligible patient to determine service needs; (2) development of an individualized integrated action plan; (3) referral and related activities to help the patient obtain needed services; (4) monitoring and follow-up; and (5) evaluation.
Essential Job Duties:
  • Connect patients with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral;
  • Complete detailed reports on needs assessments and case management activities;
  • Input accurate and complete case management data into the electronic patient database;
  • Ensure that all documents submitted on behalf of a patient are valid and authentic;
  • Assist patients in accessing the Ohio Benefits Bank and acquiring the appropriate medical, social, educational and employment resources;
  • Connect patients and families with support groups and additional behavioral health resources;
  • Assist patients in securing temporary and long-term housing and shelter;
  • Schedule treatment appointments for patients and explain the treatment timeline and objectives;
  • Ensure that patients have reliable and consistent transportation to BrightView and other care providers
  • Provide crisis interventions and de-escalation techniques;
  • Monitor and ensure that patients are seeing other health care professionals outside of BrightView when required by BrightView treatment team;
  • Assure that documentation is completed in compliance with regulatory agency requirements and best clinical practices;
  • Coordinate patient care with other BrightView team members including counselors, physicians and/or psychiatrists
  • Adhere to BrightView policies and procedures, and professional code of ethics;
  • Complete documentation utilizing the agency’s designated Electronic Medical Record, within specified timeframe;
  • Attend required meetings including but not limited to, individual and group supervision, treatment team meeting, and quarterly in-service; and
  • May provide various other services as needed including but not limited to group orientation and counseling.
Required Education/Degree:
Bachelor’s Degree preferred; Associate Degree may be considered
Required Certification/License:
LCDC II or higher preferred; CDCA with ability to advance to LCDC II may be considered
