ReNew Health, an Award winning company with established Skilled Nursing facility seeks an experienced professional to manage the facilityâs social services department and processes to ensure compliance with licensing, legal and regulatory requirements and standards of professional practice.
Working in conjunction with other members of the treatment team ensures the accurate ongoing assessment of resident psycho-social, emotional needs and the implementation of appropriate care measures.
Will provide patients and families with psychosocial support needed to cope with chronic, acute, or terminal illnesses. Utilizes consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service.
Will provide direct casework and counseling services to patient/family unit including advising family care givers, providing patient education and counseling, and making necessary referrals for other social services.
Minimum of two years skilled nursing, healthcare or long term care experience.
Minimum two years experience as a Social Services Director.
Must have Social Services Certification.
