Essential Duties and Responsibilities:

  • Coordinates and oversees alt direct and indirect patient/client services provided by clinical agency personnel.
  • Provides guidance and counseling to coordinators and clinical supervisors to assist them in continually improving all aspects of home health care services, provided through agency personnel.
  • Assists clinical supervisors in managing clinical teams and planning.
  • Provides help in assessment, planning, implementation and evaluation of patient/client and family/care giver care to all clinical personnel as indicated.
  • Assists the Executive Director/ Administrator in the preparation and administration of the agency’s budget.
  • Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses.
  • Evaluates performance of clinical supervisors.
  • Assists clinical supervisors to develop skills and techniques in evaluating the performance of clinicians.
  • Hires, evaluates, and terminates agency personnel.
  • Conducts annual evaluations on clinicians, or more frequently if indicated.
  • Oversees the maintenance of patient/client clinical records, statistics, reports and records for purposes of evaluation and reporting of agency activities.
  • Assures proper maintenance of clinical records as prescribed and in compliance with local, state and federal laws.
  • Maintains or delegates the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient/client services.
  • Develops, implements and evaluates the orientation program for new agency personnel. Orients or delegates orientation of new agency personnel.
  • Plans and implement in-service and continuing education programs to meet education and training needs of agency personnel
  • Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of home health services provided through the Organization.
  • Assists in the development of organization goals; develops, recommends, and administers organization policy and procedures.
  • Assures compliance with all local, state and federal laws regarding licensure and certification of agency personnel and, maintains compliance to the JCAHO Home Care standards.
  • Stays informed about changes in the field of nursing and home health care, shares information with appropriate agency personnel.
  • Promotes home health referrals in the health care community.
  • Demonstrates understanding of the Organization’s mission and values through behaviors that supports this.
  • In the absence of the Executive Director/ Administrator, the Director of Patient Care Services will become the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/ Administrator.
  • Plans and conducts Agency activities.
  • Conducts URIQI Committee meeting, core meeting, Team meeting every two weeks, and In-service programs.
  • Other duties as assigned by the Executive Director/ Administrator.

EDUCATION:

  • Bachelor’s Degree in Nursing required, Master’s Degree in Nursing or Public Health preferred.

EXPERIENCE/EQUIVALENT TO:

  • Ten (10) years’ experience in healthcare.
  • Three (3) to Five (5) years’ experience in healthcare management.
  • Demonstrates an ability to supervise, direct professional and administrative personnel.
  • Possess the ability to deal tactfully with the community.
  • Demonstrated knowledge of corporate business management.
  • Demonstrated knowledge of healthcare services provided to patient and family/caregiver through an interdisciplinary group.
  • Intimate knowledge of Medicare Hospice and Home Health Certification.
  • Knowledge of State, Federal and Joint Commission regulations relating to employment and employee education.
  • Proven managerial and project management experience is essential.
  • Demonstrated success in delivering results in a cross-functional team setting.
  • Strong communications, both written and verbal, presentation and facilitation skills.

LICENSES/CERTIFICATIONS:

  • State of California Registered Nurse (RN)
  • Basic Cardiac Life Support (BCLS) Card
